Governments turning to cleaner indoor air as potential gamechanger in fight against respiratory viruses

There’s a simple idea gaining traction that exponents believe is potentially transformational for society, and a win-win for public health and business. The extent to which it can deliver on its promise or not, is set to be rigorously tested. Regardless of the outcome, Australians can expect to hear a lot more about indoor air over the next few years. Scientists at the vanguard of the clean indoor air movement in this country have been struggling to be heard, until lately. A delegation of true believers recently had a meeting with Prime Minister Anthony Albanese and in Victoria, the State Government has invested $9.87 million in a Pathway to Clean Indoor Air Project. The two-year project aims to build evidence and figure out the next practical steps and best regulatory framework for improving the quality of air in schools, offices, hospitals and other buildings. The concept is uncomplicated, but achieving the desired outcomes is a little more so. The basic pitch is that humans can lead healthier and more productive lives if the air we breathe inside buildings — that we spend so much time in — is up to scratch, just like water we drink and the food we eat. These are required to meet quality standards as does outdoor air, so why not indoor air? Improved indoor air quality can reduce respiratory infections, like COVID, influenza, RSV, tuberculosis, as well as hazards such as bush fire smoke and allergens. It can also improve cognition, concentration and productivity. The Federal Government is presently reviewing the country’s handling of COVID as part of preparedness planning for the next pandemic. Indoor air is getting serious attention because it might be a vital part of our defences, by reducing the spread of respiratory diseases — both old and new. Crucially, Australia’s Chief Scientist, Dr Cathy Foley, is now on board. She commissioned a systematic review of evidence on the impact of indoor air quality on the transmission of airborne viruses in public buildings and strategies to reduce transmission. The report was presented to the National Science and Technology Council, which is chaired by the Prime Minister. Released last month, it found there were several indoor air quality strategies that could reduce the spread of airborne viruses in indoor public environments. However, there was no one strategy that can be applied to all building types. Just a few months earlier, the CSIRO teamed up with the Academy of Science and the Burnet Institute to stage an event at Parliament House in Canberra. The scientists briefed MPs on the latest research and what was happening in other countries, with the adoption of clean air regulations and standards in schools and public buildings. It was against this backdrop that Mr Albanese held a private meeting with the delegation last month. The delegation included Distinguished Professor Lidia Morawska, an Australian Laureate Fellow and described by colleagues as the world’s top aerosol scientist; Professor Brendan Crabb, who heads up the Melbourne-based Burnet Institute, a medical research body and foundation partner of the prestigious Australian Institute for Infectious Disease; Anna-Maria Arabia, chief executive of the Australian Academy of Science; and renowned anti-tobacco campaigner Dr Bronwyn King. The Burnet Institute is leading a research consortium for the Victorian project. Professor Crabb and his colleagues believe clean indoor air is the next frontier in public health, comparable to how scientific evidence and engineering promoted the adoption of clean water and sanitation in the mid-20th century. “This is not just an investment in the direct health and safety … It has the potential for improved learning, productivity and economic gains,” he said. Burnet’s technical director of the Victorian project, Associate Professor Suman Majumdar, said it was “a major step forward”. “It’s bold, it’s innovative, and it’s in line with the science,” he said. “There’s no question that these devices like portable air cleaners and germicidal UV can remove infectious aerosols from the air. That’s clear from physical and engineering sciences. “What’s not clear is how best to deploy them in various settings. It requires learning by doing. It requires engineers to put this stuff in place and figure out how best to essentially spec the devices for various settings, so they actually work.” Associate Professor Suman is the principal investigator in a groundbreaking study to assess the effectiveness of germicidal ultraviolet light in reducing the rates of COVID and other airborne viruses in residential aged-care facilities, which remain highly vulnerable to deadly outbreaks. The technology works by shining ultraviolet light across the ceiling space to create a zone of disinfection at the top of the room. Air circulation in the room will bring airborne pathogens into the disinfection zone, just below the ceiling, where the ultraviolet light can inactivate them to prevent them from causing further infections. Professor Joseph Allen, director of the Healthy Buildings Program at Harvard School of Public Health, is a leading voice in the US and globally on the issue. He’s excited to see the US Government take indoor air quality seriously, with the Centres for Disease Control and Prevention setting targets for ventilation. “If we just get the building performance right, some basics around good ventilation, good filtration, you start to address multiple threats simultaneously,” he said. “Outdoor air pollution, wildfire smoke, allergens, COVID-19, influenza, RSV, better cognitive function performance, anxiety … We have not taken advantage of the power of buildings to be a true public health tool.” According to the US Environmental Protection Agency, people, on average, spend 90 per cent of their time indoors “where the concentrations of some pollutants are often two to five times higher than typical outdoor concentrations”. “Indoor concentrations of some pollutants have increased in recent decades due to such factors as energy-efficient building construction,” it added. Professor Allen said “minor improvements to indoor air quality led to dramatic increases in cognitive function test scores”. “We estimate that there are just massive economic gains to be had,” he said. “It could lead to 10 per cent gains to the bottom-line performance of companies. “The benefits far outweigh the costs by orders of magnitude. “What would be the benefit if we said we could reduce influenza transmission indoors in schools and offices by even a small percent because we improve ventilation and filtration? Think of the (saved) hospitalisation costs, illness costs, out-of-work costs, out-of-school costs. “I always say healthy buildings are not expensive. Sick buildings are expensive.” A UK study found there was 20 per cent less illness in schools equipped with HEPA filters. WA Health is adopting a wait-and-watch approach for now. “WA Health continues to monitor new and emerging evidence about indoor air quality and the impact to health,” a spokesperson said. “During the pandemic, when there was an increased risk of transmission of COVID-19 in closed indoor spaces, relevant health advice was provided to schools and other facilities. “The Australian Health Protection Committee is carrying out work at a national level to determine the impact of indoor air quality on the transmission of airborne viral diseases in public buildings. “The outcome of this study, as well as research findings from other jurisdictions, such as the pilot project in Victoria, will be shared with WA through national committees.”
